HOW TO ENTER: New subscribers
are automatically entered. If you do not wish to subscribe write your name,
address, phone number, and TATTOO on a 3x5 index card and mail it in an
envelope to: Tattoo/CASH IS KING Sweepstakes, 28210 Dorothy Dr., Agoura Hills,
CA 91301 Limit: One 3x5 card per envelope. If you wish to subscribe, put a
check next to the titles you wish to receive on the subscription order form,
and enclose payment. Entry form must be legible and completed in full to be
valid. All entries must be received by
PRIZES AND THEIR APPROXIMATE RETAIL VALUE: One (1) Grand Prize: Grand Prize consists of $1,000 cash. Two (2) First
Prizes: two winners will receive $500 cash each; Four (4) Second
Prizes: four winners will receive $250 cash each. Ten (10) Third Prizes: ten
winners will receive $100 cash each. Twenty-five (25) Fourth Prizes: 25
individuals will receive a gift certificate from Sourpuss Clothing approximate
retail value $20 each. Fifty (50) Fifth Prizes: 50 individuals will receive a
$10 gift certificate from Sullen Clothing. All cash prizes will be issued in
the form of company checks in
SELECTION OF WINNERS: Winners will be selected by random
drawing from all entries received, and validated by Wood, Weisinger,
Sacdalan & Associates
whose decisions will be final. Drawing will be held on or about January 4, 2011
and prize winners will be notified by mail at the U.S. Mail address submitted
at time of entry within 10 days of the drawing and must acknowledge
notification within 15 days thereof, or the prize will be forfeited. THE ODDS OF WINNING DEPEND ON THE TOTAL NUMBER OF
ELIGIBLE ENTRIES RECEIVED AND ARE NOT IMPROVED BY SUBSCRIPTION PURCHASE.
ELIGIBILITY:
The Sweepstakes is
open to all residents of the
PRIZE ACCEPTANCE: ALL FEDERAL, STATE, AND LOCAL TAXES, IF ANY, ARE
THE SOLE RESPONSIBILITY OF THE WINNERS. Prize winners will receive a 1099 IRS tax from at year end
for the actual retail value of prize. No prize substitution is allowed except
by Sponsor, who may substitute a prize of equal or greater value if advertised
prize becomes unavailable. Prize winners may be required to sign an affidavit of eligibility/release of liability and publicity
within the time period designated by Sponsor, or prize may be forfeited. All
entries received become the property of Tattoo/Paisano Publications, LLC, and
none will be returned. Winners grant Tattoo/Paisano Publications, LLC and its
agencies the right to use their names and likenesses for advertising and
promotional purposes without limitation and without additional compensation,
except where prohibited by law.
